JOB DESCRIPTION:

Discharge Planning: Coordination of post hospital patient care needs from admission through discharge. Excellent communication skills to work with patients and their families to ensure safe discharge plans. Develop strong relationships with community based organizations and DME companies to ensure needed services are in place post discharge. Must be screen trained and/or willing to undergo training. Ability to lead readmission initiatives along with the readmissions team in order to reduce unnecessary readmissions for the organization.

REQUIREMENTS:

Graduate of an accredited school of nursing OR other relevant experience/education.. Knowledgeable in current medical and allied health sciences and is able to communicate effectively both verbally and in writing with medical staff and allied health professionals.
