Full job description

Description

The Registered Nurse (RN) for the Homeless Outreach Program provides compassionate, trauma-informed nursing care and health navigation services to individuals experiencing homelessness. Working as part of a multidisciplinary outreach team, the RN engages clients on the CTA Red Line to assess health needs, provide basic medical care, promote wellness, and connect individuals to healthcare and social service resources.

The ideal candidate is passionate about serving vulnerable populations, skilled in community-based care, and committed to improving health outcomes for people experiencing homelessness.

The Thresholds Homeless Outreach Program (HOP) (https://youtu.be/cyNAtMibRls?si=0F-A5IV5pb2\_tY2P) was established in 1989 to provide assertive outreach, linkage to primary and psychiatric care, and resource referral to people experiencing homelessness along with serious mental illnesses. We are Chicago’s only citywide mental health homeless outreach program, and our primary goal is simple: to get people living with mental illnesses into stable housing and help them recover and regain their lives. We believe it is not enough to simply be available to the people who need our services – we need to bring our services to them.

SCHEDULE: MON-FRI 4:00pm - 12:30am

To succeed in this role, you need:

  • Passion for mental health advocacy

  • Desire to provide compassionate care

  • Effective communication and relationship-building skills

  • Good writing skills

  • Strong time management and organization

  • A valid driver’s license, current car insurance, and daily access to your own car

  • Willingness to provide transportation to clients

Education and experience required for this role:

  • Registered Nurse license

  • Associate’s, Bachelor’s or Master’s degree from an accredited school of Nursing.
