LOCATION: Lancaster, PA

HOURS: Full Time (40 hours per week). Monday-Friday. No nights or weekends.

Penn Medicine Lancaster General Health is seeking a leader for our Care Connections & Street Medicine team to lead transformative care beyond conditional healthcare settings. The Manager drives program growth, operational excellence and community partnerships to improve health outcomes for high-risk and underserved patients.

Summary : Under the direction of the Regional Director Operations, the Manager for the Care Connection Clinic is responsible for all programmatic and administrative functions for the practices of Care Connections and Street Medicine. The Manager will have a reporting connection to the Director of Care Management for the Care Connections Program to include the staff duties/workflows and metrics. The Care Connections Manager works closely with the administrative and medical leads of the Care Connections/Street Med programs and is responsible for the coordination of duties related to program development, implementation, and outcome measurements, as well as managing the daily operation of the practice. Provides clinical consultation for internal and external customers. Coordinates and builds relationships with community-based services to enhance patients' physical and psycho-social well-being.

Responsibilities:

  • Provides strategic leadership for the Care Connections Clinic and Street Medicine programs, including program development, growth planning, forecasting, and identification of community service needs

  • Oversees daily operations of Primary Medical Care, partnering with Practice leadership to implement practice workflows, monitor quality metrics, and provide clinical oversight

  • Collaborates with ACO and Care Management leadership to define program scope, establish strategic goals, and support ongoing program development for Care Connections

  • Leads grant-related activities, including identifying funding opportunities, supporting grant applications, and overseeing grant-funded programs and resources.

  • Utilizes population health stratification methodologies to identify appropriate patient populations, support enrollment processes, and monitor program participation

  • Participates in organizational budget planning processes, including revenue review, financial monitoring, and attendance at budget-related meetings

  • Provides oversight of front-office operations to ensure accurate billing and coding practices, effective provider scheduling, and timely achievement of operational metrics

  • Promotes patient-centered care by fostering therapeutic relationships with patients and families, facilitating access to community and healthcare resources, and supporting successful transitions of care

  • Leads interdisciplinary collaboration among providers, care management teams, and community partners to ensure coordinated, high-quality patient care

  • Maintains and oversees the Care Connections recommendation and referral system to support appropriate patient engagement and resource utilization

  • Provides direct leadership for staff, including recruitment, onboarding, performance management, staff development, scheduling, and facilitation of team meetings

  • Represents Care Connections in regional and system-wide collaborative meetings, fostering partnerships and advancing strategic initiatives

  • Directs quality improvement efforts and oversees program, process, and outcome measures aligned with annual and long-term organizational goals

  • Collaborates with health system leadership to share best practices, promote innovation, and support consistency across care management programs

  • Manages contracted post-discharge services and supports care teams in securing appropriate discharge plans through partnerships with community agencies, including aging and behavioral health services

  • Performs duties in accordance with Penn Medicine and entity values, policies, and procedures

  • Other duties as assigned to support the unit, department, entity, and health system organization

Minimum Required Qualifications:

  • Master's Degree in Social Work MSW or RN with BSN.

  • RN Candidate - current RN License, issued by the PA Board of Nursing

  • Three 3 years of Community, Physician Practice or Complex Case Management experience.

  • Three 3 years of experience working with physicians, staff and executive staff

  • Three 3 years of experience leading or managing others

  • One 1 year of experience working with vulnerable populations

Preferred Qualifications:

  • One 1 year of program development experience.
