Organization: PAMF-Palo Alto Medical Foundation PAD Position Overview: Utilizes extended and expanded skills, experience and knowledge in assessment, planning, implementation, diagnosis and evaluation of the care required within an ambulatory specialty. Accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. In partnership with the care team, delivers holistic and individualized care to patients in an assigned specialty area. Develops, implements, and coordinates a plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications.

(Intended for use by Medical Foundation only) Job Description: The HF Nurse is the operational center of the Sutter Sync HF program and is responsible for working the alert queue, confirming the clinical picture, categorizing decompensation severity, coordinating multidisciplinary hand-offs, and documenting every alert, action, and event outcome. This role requires heart failure or cardiology experience and must be able to help develop and refine program workflows as the model scales, including alert operations, escalation pathways, transitional-care processes, documentation standards, and multidisciplinary hand-off routines. The role requires adaptability, sound clinical judgment, comfort with ambiguity, and the ability to translate bedside or ambulatory nursing expertise into a virtual surveillance and population-health operating model.

Nurses with the following experience are encouraged to apply: Experience developing, piloting, or improving clinical workflows, escalation pathways, patient-monitoring processes, documentation standards, or care-team operating routines in a new or evolving program.

Demonstrated adaptability, initiative, and comfort working through ambiguity, including the ability to identify workflow gaps, prioritize issues, collaborate on solutions, and adjust quickly as program needs change This role is expected to be onsite at the Sutter Innovation Center at least 2 days per week.

EDUCATION:

Graduate from an accredited School of Nursing.

CERTIFICATION & LICENSURE:

RN-Registered Nurse of California

BLS-Basic Life Support Healthcare Provider

DEPARTMENT REQUIRED CERTIFICATION & LICENSURE:

Department: OB-GYN, CNM-Certified Nurse Midwife

Department: PAMF Cardiology, ACLS-Advanced Cardiac Life Support

Department: Wound Care, CWOCN-Certified Wound Ostomy Continence Nurse

OR Department: Wound Care, WCC-Wound Care Certified

TYPICAL EXPERIENCE: 2 years of recent relevant experience.

DEPARTMENT SPECIFIC EXPERIENCE: Department: Wound Care, 2 years experience as a Wound Care RN in a clinical, active in the care of wound care patients.

Department: OB-GYN, 2 years experience as a Midwife in an acute care or clinic setting.

SKILLS AND KNOWLEDGE: Demonstrated advance knowledge and competence with regard to area of specialty, nursing theories, concepts and practices; medical terminology; anatomy; and physiology.

Advanced knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.

Possess written and verbal communication skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.

Time management and organizational skills, including the ability to prioritize patient care, assignments and work within standardized policies, procedures, and practices to achieve objectives and meet time-sensitive activities.

Basic knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record.

Work independently as well as be part of the team, while accomplishing multiple interventions in an ever-changing environment.

Ability to communicate and engage team members, patients and families in a variety of stressful situations.

Use existing procedures and critical thinking to identify and solve routine or complex problems.

Ensure the privacy of each patient’s protected health information (PHI).

Build collaborative relationships with peers and other healthcare providers to achieve departmental and corporate objectives.

Prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.

Pay Range is $86.27 to $120.77 / hour The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
