HomeWell Care Services – Carrollton is looking for a Care Manager & Community Relations Liaison to join our growing team.

This is a unique hybrid role combining care management, client support, community relations, and business development. The ideal candidate understands senior care, enjoys helping families, and is comfortable building professional relationships with healthcare and senior-care organizations.

This is not a traditional sales position. We are looking for someone who can build trust, understand care needs, represent HomeWell professionally, and develop meaningful community relationships that lead to new client opportunities.

Key Responsibilities

Care Management & Client Support

  • Meet with prospective clients and families to understand their care needs and goals.

  • Participate in client assessments and care planning.

  • Follow up with clients and families and monitor satisfaction.

  • Communicate with families, caregivers, and referral partners regarding client needs.

  • Help families understand home-care options and available resources.

  • Maintain accurate client and care-management documentation.

  • Coordinate with the HomeWell office team to ensure smooth transitions from inquiry to start of care.

Community Relations & Business Development

  • Build and maintain relationships with senior living communities, rehabilitation centers, skilled nursing facilities, hospitals, hospice organizations, physicians, social workers, senior advisors, Veterans organizations, and other referral partners.

  • Make regular in-person visits to referral sources throughout the HomeWell service area.

  • Educate community partners about HomeWell's services and how we can support their clients.

  • Identify opportunities for presentations, networking, educational events, and community partnerships.

  • Represent HomeWell at community and industry events.

  • Consistently follow up with referral sources and prospective clients.

  • Track community contacts, referrals, and opportunities.

What We're Looking For

  • 2+ years of experience in senior care, healthcare, home care, home health, hospice, assisted living, skilled nursing, social work, case management, or a related field.

  • Strong communication, relationship-building, and follow-up skills.

  • Compassionate and professional approach when working with seniors and families.

  • Comfortable making in-person community visits and networking.

  • Self-motivated, organized, and able to work independently.

  • Comfortable balancing client care responsibilities with business-development activities.

  • Valid driver's license and reliable transportation.

Preferred: RN, LVN/LPN, Social Worker, Case Manager, Care Manager, or related healthcare credentials; experience with referral development, community relations, Veterans benefits, long-term care insurance, or senior-care resources.

Success in this role means clients and families feel supported, referral partners know who to contact at HomeWell, community relationships become stronger, and new referral and client opportunities are consistently developed.

Compensation & Schedule

Salary: Starting at $60,000 annually, depending on experience and qualifications, plus performance-based incentives.

Position: Full-Time Location: Carrollton, TX / DFW Metroplex Schedule: Primarily Monday–Friday, with occasional community events outside normal business hours. Local travel throughout the HomeWell service area is required.
