Description

Concern Housing is seeking a Financial Case Manager to provide financial management services to tenants/ residents residing in Concern’s NYC housing programs. The role is multi-focused and includes assistance with rent collection and monitoring, establishing and maintaining eligibility for benefits, and providing budgeting education and support to tenants to help improve their financial literacy skills. The position also serves as Concern’s NYC representative payee liaison with the Social Security Administration (SSA) and agency contact with the Human Resources Administration (HRA).

Days/Hours: Monday - Friday; 8am - 4pm

Duties and Responsibilities:

  • Provide case management services, including service planning, medication monitoring, referrals, community integration, and entitlement support.

  • Provide individual counseling, support services, and required documentation.

  • Assist clients with housing needs, maintenance concerns, landlord and lease issues, and rent related matters.

  • Conduct site visits and monthly fire safety inspections as required.

  • Assist clients with benefits, financial assistance, appeals, and recertifications.

  • Coordinate Representative Payee services, including SSA documentation, benefit payments, rent, bills, and PNA distributions.

  • Monitor rent payments and arrears in NetSuite, follow up on outstanding balances, and establish repayment agreements as needed.

  • Provide financial literacy, budgeting, and money management support.

  • Participate in weekly supervision and staff meetings.

  • Provide crisis intervention, including 24/7 cell phone coverage.

  • Maintain required documentation and remain current with all agency compliance requirements and trainings.

Requirements: Bachelors Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 2 years experience working in a similar setting and/or working with people with mental illnesses. Demonstrated knowledge of community resources, including Social Security, Public Assistance, and Medicaid insurance systems. NYS motor vehicle license required.
