Lallie Kemp Regional Medical Center is seeking aSocial Service Counselor 1-2 to join our dedicated team. Lallie Kemp Regional Medical Center is a 24-bed acute care critical access hospital serving the Florida Parishes region of Southeast Louisiana. We offer a wide range of services including general surgery and a 24-hour emergency department. At Lallie Kemp Regional Medical Center, we are able to bring you a wide range of services close to home, provided by a network of board certified physicians.
This Social Service Counselor 1-2 position is located in the Social Services Department
AN IDEAL CANDIDATE SHOULD POSSESS THE FOLLOWING COMPETENCIES:

Minimum Qualifications
Three years of social services experience; OR
Six years of full-time experience in any field; OR
A bachelor's degree.
EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.
NECESSARY SPECIAL REQUIREMENT:
Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor.

Job Duties and Other Information
Job Duties:

Conduct interviews with patients and families to obtain social history. Assess patients to determine patient's motivation, abilities, job skill potential. and if there is a need for financial assistance, transportation, food stamps, in-home aide, and/or any other community resources to aid in discharge planning Consult with attending physicians, the multi-disciplinary team and other health care professionals regarding community resources.

Responsible for specialized caseloads including: Swing bed patients: perform assessment on all swing bed patients. Completes activities assessment within 24 hours of admission and oversees and schedule recreational activities for the swing patients daily, provides plan for weekend staff to carry out recreational activities

Participate on the Multi-Disciplinary Discharge Planning Treatment Team daily.

Coordinate referrals initiated for families and patients requiring assistance. Possible referrals for the inpatient, outpatient, and emergency room are as followed: Home Health, Mental Health, Nursing Home, Hospice, Rape, Detox Treatment, Protective services, Durable Medical Equipment, Oxygen, Medical Assistance, Financial Assistance, Transportation, and any other eligible community resources required.

Assist patients in Medication Procurement, as needed.
Position Specific Details:
Work Location: This position is located in the Social Services Department.
Appointment Type: Probational
Work Schedule: The shift schedule will be determined by the Department Manager and the incumbent must be available to work on-call as needed.
Career Progression: As part of a Career Progression Group, vacancies may be filled from this recruitment as a Social Services Counselor 1 or 2 depending on the level of experience of the selected applicant(s).
Compensation: The following is the rate of pay for the Social Services Counselor 1 or 2 Vacancy:

Social Services Counselor 1 – Starting Salary is $16.37/per hour worked, and On-Call Pay.

Social Services Counselor 2 – Starting Salary is $17.52/per hour worked, and On-Call Pay.
