Position Overview

The Dialysis Social Worker supports both in-center and home dialysis patients, helping individuals and families navigate the social, emotional, financial, and practical challenges associated with chronic kidney disease and dialysis treatment.

This role is a vital member of the interdisciplinary care team and works closely with physicians, nurses, dietitians, and other healthcare professionals to develop and support comprehensive patient care plans.

What You'll Do

  • Provide social work services to both in-center and home dialysis patients.

  • Participate in monthly patient care rounds with the interdisciplinary team.

  • Attend dialysis clinic visits with physicians and nursing staff.

  • Assist patients with transportation resources and standing transportation arrangements.

  • Conduct patient quality-of-life assessments and screenings.

  • Help patients establish and maintain advance directives.

  • Connect patients with community resources and financial assistance programs.

  • Address social, emotional, environmental, and personal barriers that may impact patient care and treatment success.

  • Advocate for patients and families while supporting positive healthcare outcomes.

  • Build trusted relationships with patients and serve as an ongoing source of support throughout their dialysis journey.

Required Qualifications

  • Master's Degree in Social Work (MSW).

Preferred Qualifications

  • Previous dialysis experience preferred.

  • Healthcare or medical social work experience preferred.

  • Experience working with diverse patient populations.

Schedule Information

This position requires some scheduling flexibility.

Lynchburg Dialysis operates one of the area's largest dialysis programs and is the only local unit that provides a third-shift dialysis service. Occasional flexibility may be needed to support patients receiving treatment during extended operating hours (typically until 6:30 PM on extended days). Flexible scheduling and adjusted work hours are available to support coverage needs.

MINIMUM REQUIREMENTS

Education: Master's degree in Social Work from a CSWE accredited social work program required.

Experience: No relevant experience required; Experience in the social work field; experience in health care settings is preferred.

Licensure: Expectation to become Licensed Clinical Social Worker (LCSW) guidelines found in departmental addendum located in department competency file.

PHYSICAL DEMANDS

Job requires sitting for prolonged periods, standing/traveling or use of assistive and climbing (stairs, steps). Proficient communicative, auditory and visual skills; Attention to detail, hear, speak, see, distinguish colors, read, ability to write legibly; Ability to lift/push/pull less than 20lbs. May be exposed to chemicals, blood/body fluids and infectious disease.

The pay range for this role is $57,907.20 - $92,643.20 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
