Assesses patient’s physical, psychosocial, cultural, and spiritual needs through observation, interview, review of medical records, and clinical data. Evaluates patient’s response and reports to responsible provider to ensure optimal patient outcome. Contributes to the development and implementation of patient care plans in collaboration with the interdisciplinary team. Demonstrates competency in therapeutic and diagnostic interventions, including vital signs, minor treatments/procedures, prevention services, pre-visit planning, post-visit follow-up, and medication refills per protocol. Promotes, advocates, and collaborates to protect the health, safety, and rights of each patient. Provides patient and family education through assessment of health literacy, barriers to learning, readiness, needs, and ability. Encourages patient engagement in healthcare partnership using motivational interviewing, evidence-based self-management support protocols, tools, and teach-back methodology. Aids in the coordination of the patient’s care plan. Recognizes the patient or designee as a full partner in providing compassionate and coordinated care based on respect for the patient’s preferences, values, and needs. Provides relationship-based care, understanding what is most important to the individual and actively engaging them in all aspects of care. Contributes to continuous process improvement by taking an active role in patient experience and satisfaction. Other duties as assigned. Oversees clinic staff as assigned, ensuring compliance with policies and procedures, preparing work schedules, ensuring adequate staffing, resolving clinical operational problems, managing clinical supply ordering and expenses, and conducting performance reviews.Knowledge, Skills, and Abilities:
