Overview

As the Director of Care Management, you will play a pivotal role in ensuring the delivery of high-quality patient care and effective care coordination within our primary care setting. You will lead a team of care managers and other care team support members and collaborate closely with healthcare providers to optimize patient outcomes and enhance the overall patient experience.

Responsibilities

  • Develop and implement care management strategies and protocols to promote effective patient care delivery and care coordination within the primary care setting.

  • Lead and supervise a team of care managers, providing guidance, mentorship, and support to ensure high performance and professional development.

  • Collaborate with healthcare providers, including Physicians, Nurse Practitioners, and allied health professionals, to streamline care processes, enhance communication, and optimize patient outcomes.

  • Oversee the assessment, planning, implementation, and evaluation of patient care plans, ensuring adherence to evidence-based practices and regulatory standards.

  • Utilize data analytics and performance metrics to monitor and evaluate the effectiveness of care management programs, identify areas for improvement, and implement quality improvement initiatives.

  • Foster a culture of patient-centered care, empathy, and collaboration among care management staff and healthcare providers.

  • Develop and maintain strong relationships with external stakeholders, including payers, community organizations, and other healthcare providers, to facilitate seamless care transitions and coordination across the care continuum.

  • Stay abreast of industry trends, best practices, and regulatory changes in care management and population health management and incorporate relevant knowledge into program development and implementation.

  • Collaborate with the leadership team to develop strategic initiatives and goals aligned with the company's mission, vision, and values.

Qualifications

  • Bachelor's degree in nursing required, Master's degree in a related health field preferred

  • Proven experience in care management, care coordination, or population health management within a primary care setting.

  • Current licensure as a Registered Nurse (RN) required, compact state licensure preferred

  • Demonstrated leadership abilities, with a track record of effectively leading and developing high-performing teams and delivering results for all.

  • Strong interpersonal, communication, and collaboration skills, with the ability to build rapport and relationships with diverse stakeholders.

  • Excellent analytical and problem-solving skills, with the ability to interpret data and implement data-driven solutions.

  • Knowledge of healthcare regulations, quality standards, and best practices related to care management and population health management.

  • Ability to thrive in a fast-paced, dynamic environment and adapt to changing priorities and challenges.

  • Commitment to continuous learning and professional development in the field of healthcare management and patient care.
