Cleveland Clinic’s Business Operations Center serves as a key hub for our Home Care and Hospice operations as well as many administrative and support teams. This collaborative workplace supports a wide range of caregivers, clinical professionals, and operational teams. While many positions are based within the center, numerous home-visiting roles—including case managers, nurses, and therapists—travel throughout the Greater Cleveland and Akron/Canton areas to deliver exceptional care directly to patients in their homes.
We are looking to add an experienced Physical Therapist to our team of caregivers to treat patients throughout Medina, Stark, Wayne, Summit and Portage Counties. You will conduct and evaluate medically prescribed physical therapy treatment programs for our patients from the comfort of their own homes. Most patients treated in this area need orthopedic care, with the remainder being Parkinson’s, COPD and Multiple Sclerosis patients.
This is a PRN role. A caregiver in this position will work varying hours and days. Weekend rotations occur 1 Saturday every 5 weeks and 1 Sunday every 10 weeks. 1 holiday per year is required. PRN caregivers are expected to attend 2 weeks of full-time orientation.
A caregiver who excels in this role will:

Perform initial and ongoing assessments of patient's condition.
-
Establish, revise, and evaluate a plan of care which is appropriate to problems identified and involves the patient/family according to their capabilities and desires.

Perform therapy interventions using standard physical therapy techniques and skills appropriate for the patient's age and condition.
-
Supervise activities of physical therapy assistants, students, and other support personnel.
-
Maintain complete and up-to-date documentation and files regarding patient condition and effectiveness and timeliness of care and interventions.

Participate in ongoing interdisciplinary assessment of assigned patients.
-
Participate as a member of the hospice interdisciplinary team when necessary.
Minimum qualifications for the ideal future caregiver include:

Bachelor's Degree or certificate
-
Graduate of a Physical Therapy Program accredited by the American Physical Therapy Association
-
Valid Ohio Driver’s license upon hire; per policy 8-175 thereafter
-
Proof of automobile insurance coverage with $100,000/$3000,000 coverage, upon hire; per policy 8-175 thereafter

Current State licensure as a Physical Therapist (PT)
-
Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or or the American Red Cross
Preferred qualifications for the ideal future caregiver include:
- One year of clinical experience as a licensed physical therapist in an acute, rehab or skilled nursing setting
Physical Requirements:
- Ability to walk and climb stairs without assistive device and to maintain balance despite sudden load in order to support patient from falling.
- Ability to meet physical demands characterized at the heavy level defined by 1993 Leonard Matheson and Ministry of Labor.
- Ability to lift and reach with arms sufficiently to lift patient's arm and leg during therapeutic treatment program.
- Sensation of touch and manual dexterity to be able to palpate muscle tension or to monitor patient pulse.
- Visual acuity to observe patient's gross and fine motor movements in order to analyze these movements and to read all patient monitors.
- Ability to speak clearly enough to instruct patients and families.
- Ability to hear at a conversational level to understand patient's description of symptoms and response to treatment.
Personal Protective Equipment:
- Follows standard precautions using personal protective equipment as required.
