Job Title: Care Coordinator

Job Description

The Care Coordinator supports a high-volume, multi-specialty medical clinic by managing patient intake, scheduling, insurance verification, and ongoing communication between patients, providers, pharmacies, and laboratories. This role ensures patients receive timely, accurate, and compassionate support both in-person and through telehealth, while maintaining precise documentation in the electronic medical record system.

Responsibilities

  • Perform patient intake and check-in, ensuring all required information is collected accurately and efficiently.

  • Verify patient insurance coverage and benefits prior to visits and document findings in the system.

  • Schedule a high volume of patient appointments each day, following approved scheduling protocols and ensuring the appropriate provider is assigned for each visit type.

  • Handle a large volume of incoming patient calls within a seven-person department, contributing to the management of approximately 600 calls per day.

  • Use the Jelly Bean feature in the electronic system to review and process electronic refill requests from outside pharmacies.

  • Schedule patients correctly based on the type of visit requested, confirming that the appropriate doctor is scheduled in accordance with established guidelines.

  • Respond promptly and professionally to patient portal messages, providing information, updates, and follow-up as needed.

  • Complete keying and entry of patient information in a timely and efficient manner, maintaining accuracy and confidentiality.

  • Make changes to physician schedules as needed and generate monthly appointment schedules to support clinic operations.

  • Collaborate closely with patients, providers, pharmacies, and laboratories to coordinate patient orders and ensure continuity of care.

  • Maintain and display professionalism and strong customer service skills throughout the day in all patient and team interactions.

  • Demonstrate flexibility in job duties and functions, adapting to changing priorities and clinic needs.

  • Communicate effectively with a geriatric patient population, using patience, clarity, and empathy.

  • Utilize strong technology and video conferencing skills to support clinic operations.

  • Facilitate and guide patients through telehealth appointments, ensuring they can connect and participate successfully.

  • Work within the EMR system (E Clinical Works) to document patient encounters, update records, and manage scheduling and orders.

  • Assist with filling prescription requests in coordination with providers and pharmacies, following established protocols.

  • Apply medical terminology and medication or pharmacy knowledge when communicating with patients and healthcare partners.

  • Provide excellent customer service to patients in urgent care, internal medicine, pediatrics, and other clinic specialties.

Essential Skills

  • 12 years of recent medical office experience.

  • Current BLS and/or CPR certification.

  • Knowledge of medical terminology.

  • Medication knowledge or pharmacy knowledge.

  • Experience working with electronic medical records (EMR), including E Clinical Works.

  • Strong computer skills and ability to learn new systems quickly.

  • Experience with insurance verification and benefits confirmation.

  • Proficiency in scheduling patient appointments in a medical setting.

  • Strong phone etiquette and ability to manage high call volumes.

  • Strong communication skills with patients, providers, pharmacies, and laboratories.

  • Ability to work effectively with geriatric patients and diverse patient populations.

  • Detail-oriented approach to data entry, scheduling, and documentation.

  • Ability to facilitate and support telehealth appointments using video conferencing tools.

  • Customer service experience in a medical, urgent care, or internal medicine environment.

Additional Skills & Qualifications

  • Experience in medical assisting or patient services.

  • Experience in urgent care, internal medicine, pediatrics, or radiology settings.

  • Familiarity with EKG procedures.

  • Prior experience in a high-volume medical clinic or multi-specialty practice.

  • Comfort working with multi-specialty teams and supporting multiple providers.

  • Interest in professional development opportunities such as training roadmaps and online learning platforms.

  • Ability to adapt to a family-oriented, tight-knit team culture.

Work Environment

The role is based in a high-volume medical clinic that provides services in pediatrics, urgent care, adult care, and radiology, and supports approximately 17 doctors. The clinic can see 250 or more patients per day, creating a fast-paced and dynamic environment. The standard work schedule is Monday through Friday from 8:15 a.m. to 5:15 p.m. You will work extensively with an electronic medical record system (E Clinical Works), phone systems, and video conferencing tools to support both in-person and telehealth visits. The culture is very family-oriented, tight knit, and welcoming, with a strong emphasis on teamwork and supporting a diverse patient community. The clinic is located within one mile of several restaurants and amenities, offering convenient options for breaks and meals.

Job Type & Location

This is a Contract to Hire position based out of Fountain Valley, CA.

Pay and Benefits

The pay range for this position is $20.00 - $23.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
