Department: St. Joseph Medical Group- Administration
Shifts Available: Days
Employment Type: Full-Time
Hours: 8:00 am-5:00 pm (8-hour shifts)
Location: St. Joseph Medical Center—Kansas City, MO
We are seeking an LPN Care Manager to support outpatient clinic operations with a focus on care coordination, case management, population health, chronic disease management, and transitions of care. This role partners with providers to improve patient outcomes, reduce readmissions, and support value-based care initiatives.

Responsibilities
Essential Duties and Responsibilities (includes, but is not limited to):

Partner with interdisciplinary teams to achieve care coordination, quality, and financial performance objectives.

Lead and manage in-person and virtual meetings with practice managers, staff, providers, and care managers to share results and identify improvement opportunities.

Stay current on payor and regulatory requirements and communicate updates effectively.

Manage high-risk patients and support chronic disease management (diabetes, CHF, COPD)

Document in EMR systems and support quality/HEDIS measures

Provide patient education and remove barriers to care

Coordinate patient care plans, referrals, and follow-ups
Qualifications
Before we go any further, we do have some deal-breakers. You must have:

Current and valid state license as an LPN.

Current BLS (AHA) certificate upon hire and maintain current.

A minimum of one year of experience in an applicable setting is preferred.

IV certification upon hire.
