Overview

Department: St. Joseph Medical Group- Administration

Shifts Available: Days

Employment Type: Full-Time

Hours: 8:00 am-5:00 pm (8-hour shifts)

Location: St. Joseph Medical Center—Kansas City, MO

We are seeking an LPN Care Manager to support outpatient clinic operations with a focus on care coordination, case management, population health, chronic disease management, and transitions of care. This role partners with providers to improve patient outcomes, reduce readmissions, and support value-based care initiatives.

Responsibilities

Essential Duties and Responsibilities (includes, but is not limited to):

  • Partner with interdisciplinary teams to achieve care coordination, quality, and financial performance objectives.

  • Lead and manage in-person and virtual meetings with practice managers, staff, providers, and care managers to share results and identify improvement opportunities.

  • Stay current on payor and regulatory requirements and communicate updates effectively.

  • Manage high-risk patients and support chronic disease management (diabetes, CHF, COPD)

  • Document in EMR systems and support quality/HEDIS measures

  • Provide patient education and remove barriers to care

  • Coordinate patient care plans, referrals, and follow-ups

Qualifications

Before we go any further, we do have some deal-breakers. You must have:

  • Current and valid state license as an LPN.

  • Current BLS (AHA) certificate upon hire and maintain current.

  • A minimum of one year of experience in an applicable setting is preferred.

  • IV certification upon hire.
