Job Description

A client of Insight Global in the healthcare technology sector is seeking a Program & Care Coordinator to help grow and scale a grant funded health initiative across the state of Tennessee. This program is designed to engage at risk population for heart disease and enroll them into this program to give them access to a wearable ECG device to catch heart disease early and save lives. This role owns what happens after a practice is engaged & signs into the health program. This includes getting them implemented, keeping them supported, and making sure every patient who enrolls actually moves through screening, results, and follow-up. It is half practice-facing and half patient-facing, and both halves matter equally. For practice support the duties include:

  • Own site onboarding for every participating practice: readiness assessment, workflow mapping, and an implementation plan adapted to rural primary care

  • Deliver device and portal training to PCPs, nurses, and administrative staff, including live demonstrations against active patient schedules

  • Map the program into each clinic's existing workflow — device placement, report routing, referral triage — and measurably reduce administrative burden

  • Serve as the standing point of contact for participating practices, resolving issues before they reach the Program Director

  • Support the Program Director in recruitment conversations, bringing the clinical credibility that turns practice interest into commitment For care coordination this role includes:

  • Track every enrolled patient from screening through result, specialist input, and follow-up, so no one falls out of the funnel

  • Coordinate the Heartbeat Health escalation path — expert review, eConsult, and cardiologist-to-PCP consult — and keep turnaround inside SLA

  • Ensure urgent findings reach the treating PCP immediately, with the escalation documented

  • Help patients navigate next steps: scheduling, what their results mean, and what happens after a referral

  • Keep patients on defined re-monitoring cadences, care navigation, and support retention across five-year program and follow-up

Skills and Requirements

  • Clinical background required — RN, LPN, PA, NP, paramedic, or equivalent clinical training and licensure. You need to be credible with a rural PCP from the first conversation.
  • 3-5 years in clinical operations, medical device or digital health implementation, clinical account management, or a practice-facing healthcare role
  • Comfort recruiting and building relationships with independent practices — this role opens doors as often as it walks through them
  • Demonstrated ability to deliver in-person and virtual training to clinical and administrative audiences, with strong process documentation habits
  • Willingness to travel regularly to rural Tennessee counties
  • Cardiovascular or ambulatory monitoring experience is a plus, but we care most about your ability to earn trust inside a small practice
