Evening Shift Licensed Practical Nurse (LPN) Rate: $28.00 - $32.58/hour (Shift Differential included)Sign-on bonus: 2nd shift up to $5,000!Shift: 1st and 3rd shift up to $3,000!
Chautauqua Nursing & Rehabilitation Center is looking to add a Licensed Practical Nurse (LPN) to its nursing team. RESPONSIBILITIES:

Provide nursing care in accordance with resident assessments, care plans, and facility policies
-
Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice
-
Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties
-
Accurately document care, treatments, observations, and medication administration
-
Supervise and support Nursing Assistants to ensure quality resident care
-
Maintain up-to-date resident care information and communicate pertinent information during shift changes
-
Follow infection control, safety, resident rights, and confidentiality standards
-
Participate in interdisciplinary care planning and quality improvement initiatives
-
Assist with admissions, transfers, and other nursing responsibilities as assigned
REQUIREMENTS:
-
High School Diploma or equivalent required; Associate's Degree preferred

Valid NYS Licensed Practical Nurse (LPN) License required

Current CPR Certification required
-
Experience in a skilled nursing facility or with the geriatric population preferred

Strong clinical, communication, and organizational skills
-
Knowledge of medication administration, infection control, and resident care practices

Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team
