Job Description

A Passionate Helping Hand Home Health Care is seeking an experienced Clinical Director to lead all clinical operations and ensure exceptional patient care across our home health organization. This leadership role is responsible for clinical oversight, regulatory compliance, quality improvement initiatives, staff development, and operational excellence. The ideal candidate is a strong clinical leader with deep home health experience who can drive positive patient outcomes, maintain survey readiness, and support a culture of accountability, compassion, and continuous improvement. Day-to-Day Responsibilities

  • Provide leadership and oversight for all clinical operations and patient care services.
  • Supervise and mentor RNs, LPNs, therapists, social workers, home health aides, and contracted clinicians.
  • Ensure compliance with Medicare, Medicaid, CMS Conditions of Participation, accreditation standards, HIPAA, and state regulations.
  • Lead Quality Assurance and Performance Improvement (QAPI) programs, including audits, outcomes tracking, and corrective action plans.
  • Oversee OASIS accuracy, clinical documentation quality, physician orders, care plans, and survey readiness activities.
  • Monitor key quality metrics, including hospitalization rates, patient satisfaction, documentation compliance, and clinical outcomes.
  • Partner with billing, authorization, revenue cycle, and administrative teams to reduce claim denials and support reimbursement.
  • Facilitate clinical meetings, case conferences, staff education, and competency development programs.
  • Ensure timely completion of patient assessments, care plans, recertifications, discharges, and compliance documentation.
  • Utilize Axxess and other EMR systems to oversee clinical performance, documentation, and operational reporting.

Skills and Requirements

  • Current Registered Nurse (RN) license.
  • 3+ years of home health experience.
  • 2+ years of clinical leadership, management, or supervisory experience.
  • Strong knowledge of Medicare and Medicaid home health regulations.
  • Experience with CMS Conditions of Participation and home health compliance requirements.
  • Hands-on experience with OASIS assessments and documentation.
  • Experience preparing for surveys, audits, and regulatory reviews
  • Excellent leadership, communication, organizational, and coaching skills.
  • Experience working within electronic medical record (EMR) systems.
  • Valid driver's license and reliable transportation.
  • ACHC accreditation or survey experience.
  • Home health regulatory experience within Indiana.
  • Experience leading QAPI initiatives and quality improvement programs.
  • Revenue cycle, reimbursement, and documentation improvement experience.
  • Experience with authorizations and utilization management.
  • Proven success reducing claim denials and improving clinical documentation quality.
  • Experience leading interdisciplinary healthcare teams.
  • Experience with Axxess Home Health or similar home health EMR platforms
