Registered Nurse Care Coordinator (RNCC) - Cardiac and Vascular Surgery Clinic

As a Registered Nurse Care Coordinator in UVA Health's Cardiac & Vascular Surgery Clinic, you will support a growing, high-acuity practice that cares for patients undergoing advanced cardiac and vascular procedures. The Role

The Cardiac & Vascular Surgery Clinic is a highly specialized surgical practice supporting both cardiac and vascular surgery programs. Working alongside surgeons, nurse navigators, RNs, and multidisciplinary specialists, you will help coordinate care across the surgical continuum while serving as a key resource for patients and families navigating complex treatment plans.

In a typical day, you will:

• Coordinate care for cardiac and vascular surgery patients from referral through recovery.

• Partner closely with assigned cardiovascular surgeons to support both clinical and operational workflow needs.

• Review referrals, facilitate care plans, and guide patients through surgical pathways.

• Coordinate testing, pre-surgical workups, and required documentation to ensure patients are fully prepared for surgery.

• Support post-operative follow-up and help address care coordination needs during recovery.

• Utilize telehealth and virtual care tools to enhance patient access and continuity of care.

• Collaborate with colleagues across Cardiology, Vascular Medicine, Interventional Radiology, Structural Heart, and TAVR programs.

• Participate in a strong cross-coverage model that promotes teamwork, continuity, and shared success.

This role offers exposure to an exceptional range of complex cardiovascular populations, including cardiac surgery, vascular surgery, aortic disease, valve surgery, TAVR, adult congenital heart disease, heart transplant patients, and robotic cardiac surgery . Unlike many traditional ambulatory nursing roles, this position allows you to focus on highly specialized surgical patient management in a streamlined and impactful practice setting.

MINIMUM REQUIREMENTS:

Education : Bachelor of Science in Nursing from an accredited nursing program.

Experience : 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.

Specialty Board Certification strongly preferred

License/Certification: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently traveling, and bending/stooping. Proficient communicative, auditory, and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.

The pay range for this role is $85,820.80 - $113,401.60 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
