Full job description
Adecco Healthcare & Life Sciences is seeking an experienced Registered Nurse (RN) for a Home Health contract in Amherst, New York . This is an excellent opportunity to join a patient-focused team while making a meaningful impact in the community.
Job Details

Coverage Area: Erie County
-
Contract Length: Three-month contract, possibility of an extension
-
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
-
On-Call Requirement: One week per month, Fri - Fri
-
Pay Rate: $53.00 - $55.90 per hour, mileage reimbursement
Responsibilities
-
Provide skilled nursing services in patients' homes throughout the assigned coverage area
-
Manage patient caseloads and coordinate ongoing care
-
Complete comprehensive patient assessments and documentation
-
Develop, implement, and monitor individualized care plans
-
Collaborate with physicians and interdisciplinary care teams
-
Educate patients and caregivers on treatment plans and disease management
-
Ensure accurate and timely documentation of all patient care activities
Requirements
-
New York Registered Nurse (RN) license
-
Home Health experience required
-
Strong case management experience
-
Ability to work independently in the field
-
Reliable transportation for local travel
-
Strong assessment and documentation skills
Preferred Skills
-
Experience with HCHB (Homecare Homebase) documentation
-
Strong care coordination and case management skills
-
Excellent communication and organizational abilities
-
Experience managing a home-based patient population

Pay Details: $53.00 to $55.90 per hour
