Job Duties

University of Minnesota Physicians is seeking a Certified Nurse Practitioner or a Certified Physician Assistant who will deliver care for patients on the University of Minnesota Heart Care- Heart Failure team at the University of Minnesota Medical Center and the Clinics and Surgery Center (CSC) in Minneapolis. The position will provide patients with expert patient care and coordination of services. They will assist the practice to improve patient satisfaction and enhance the quality of care provided to patients through increased access and coordinated services.

In This Role, You Will:

Join a multidisciplinary team of 40 Cardiologists- 9Heart Failure focused, and 22 Advanced Practice Providers-5 Heart Failure focused.

Provides and coordinates the deliveryofcare for adults with Heart Failure (HF) including heart transplantation patients and mechanical circulatory support (MCS) therapies.

The NP/PA performs independent follow-up of patients in the clinic setting.

Participates in multidisciplinary advanced heart failure rounds in the inpatient setting, perform and documents inpatient assessments and formulates a plan to address complex, acute, and chronic health needsto advance the plan of care. Includes ordering appropriate tests, labs, and interpretation of results.

Able to be an effective team member and work well with others in HF team and broader Cardiology/Medical practice/Hospital staff.

Able to communicate well with other health care personnel. Able to communicate and arrange for hospital admission and discharge, as appropriate.

Strong understanding of patient HF home education needs and commitment to exemplary patient care.

Participate in HF, transplant, andMCS meetings as needed. Excellent and compassionate communicator to patients and family members.

Manages patient care through appropriate use of available resources.

Demonstrates ability to deliver care or service adjusting approaches to reflect developmental level of population served.

Utilize a fully integrated Electronic Medical Record (EPIC)

Your Schedule:

0.9 FTE; 10-hour days

Rotating hospital and clinic coverage

Rotating weekend coverage on the inpatient service

We're Looking For a PA or NP Who Is:

A graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program.

Eligible or is certified through NCCPA, ANCC or AANP.

Licensed or eligible to acquire a license and prescriptive authority through the MN Board of Medical Practice (PAs) or MN APRN license or eligible to acquire a license and prescriptive authority through MN Board of Nursing (NP's).

Able to meet threshold criteria/qualifications for credentialing and privileges.
