Patient Care Coordinator (Sat/Sun)

This position requires an active, unrestricted North Carolina nursing license.

As a Patient Care Coordinator, you will:

• Develop/maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on Company's services available in a market.

• Ensure collaboration with program staff and other field sales resources

• Assist program in processing of physician orders

• You will report to a Market Executive of Sales

This is a part time position and the schedule will be a 9 hour shift every Saturday and Sunday

Candidates must live within an hour of Delco, NC and be available to attend weekly sales meetings as required.

Required/Preferred Experience/Skills:

• North Carolina RN/LPN license required.

• Bachelor's of Science in Nursing preferred.

• 3 years clinical experience preferred.

• Must have knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.

• Excellent customer service, account development capabilities, organization, problem-solving and communication skills

• This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$59,300 - $80,900 per year
