Salary: $104,000 USD per year

Description

Wound Care RNSalaried: $104,000Hourly Equivalent: $50/hour Shift: Full-TimeSchedule: Monday- Friday, 8 am - 4 pm

Brookhaven Health Care Facility is looking to add a Wound Care RN to its clinical team. In this role, you'll provide specialized wound care services while supporting resident healing, infection prevention, and overall quality outcomes. RESPONSIBILITIES:

  • Provide direct nursing care to residents in accordance with care plans, physician orders, and wound care protocols

  • Assess, monitor, and document wound status, treatment effectiveness, and resident progress

  • Administer medications, IV therapies, treatments, and wound care interventions as prescribed

  • Collaborate with physicians, nursing staff, therapy teams, and other interdisciplinary team members to support resident recovery and care goals

  • Develop, implement, and update wound care plans as needed

  • Monitor for signs of infection, skin breakdown, and changes in resident condition, reporting concerns promptly

  • Supervise and support CNA and LPN staff to ensure quality resident care

  • Maintain accurate documentation, assessments, care plans, and clinical records

  • Follow infection control, resident safety, and HIPAA guidelines

  • Perform additional duties as assigned

REQUIREMENTS:

  • Valid New York State Registered Nurse (RN) License required

  • CPR Certification required or obtained upon hire

  • Experience in skilled nursing, rehabilitation, long-term care, acute care, or wound care preferred

  • Strong clinical assessment, wound management, and critical thinking skills

  • Ability to supervise, delegate, and collaborate effectively with interdisciplinary teams

  • Excellent communication, organizational, and documentation skills

  • Commitment to providing compassionate, resident-centered care
