Responsibilities

The Manager of Specialty Programs is responsible for the internal implementation and external growth of specialized clinical programs. This position leadsclinical excellence and brings the right balance of operational oversight, fiscal responsibility, teamwork and hands-on leadership, ensuring that clinicians are supported, care transitions are seamless, patients experience continuity across the entire care continuum - both internal and external, and that our care partners are supported.This role ensures these programs comply with regulatory standards, meet operational key performance indicators and deliver high-quality, cost-effective patient care.

  • Lead the operational rollout and day-to-day oversight of specialty care models across the agency's service lines.

  • Partners with clinical leaders to driving care continuity and transitions with a focus of clinical excellence at the point of care. Works to move patients seamlessly through continuum of care.

  • Contributes to the development of annual budget; monitor patient revenues, clinical, and operational expenses and staff productivity. Reports variance to Director and takes appropriate actions to meet or exceed budget targets.

  • Monitors key performance indicators and creates an action plan to improve results.

  • Effectively manages employees by hiring, onboarding, managing performance, and supporting growth and development. Provides frequent in-field shadowing of direct care staff.

  • Drive process improvement and change management activities to drive continuous improvement in patient outcomes within the specialty programs.

  • Ensure clinician documentation is timely, accurate, and meets payor or Medicare requirements.

  • Serve as a clinical and operational resource, managing relationships with physicians, hospitals, and community stakeholders.

Qualifications

REQUIRED QUALIFICATIONS

  • Bachelor’s Degree requiredor each year of approved experience may be substituted for one year of educationin nursing from an accredited college or university.

  • Two (2) years of experience in a direct supervisory role required.

  • Must have excellent interpersonal, communication and analytical skills.

  • Software skills: Electronic medical record, Microsoft Excel, Word, Outlook, PowerPoint, etc.

  • Program management or leadership experience in a home health or hospice setting, preferred

  • Understanding of Medicare/Medicaid conditions of participation, transitional care, and clinical appropriateness guidelines, preferred

Required Licensure/Certifications

  • Must hold at least one active unencumbered RN license in either unencumbered VT or NH at the time of hire. Employees must have active unencumbered RN licenses in both VT and NH within 30 days of their hire date.

  • Valid driver’s license with a clean driving record

  • Current car insurance, which meets minimum standards

  • BLS within 90 days of hire

  • Area of Interest:Professional/Management

  • Pay Range:$35.00/Hr. - $61.75/Hr.

  • FTE/Hours per pay period:1.00 - 1.00 - 40 hrs/week
