Salary Range: $27.48 To $28.85 Hourly
POSITION SUMMARY:
The Clinical Quality Improvement Specialist supports Syracuse Community Health's quality improvement, population health, and value-based care initiatives by integrating clinical expertise with performance improvement and data analysis. This position analyzes clinical and operational performance, conducts detailed chart reviews, validates quality measure performance, and identifies opportunities to improve patient outcomes, regulatory compliance, and organizational performance. Working collaboratively with providers, clinical staff, and operational leaders, interprets clinical guidelines, quality measures, and documentation requirements to develop actionable recommendations and support evidence-based decision-making. The role serves as a clinical and analytical resource, using critical thinking to identify performance gaps, evaluate workflows, determine root causes, and support the implementation of sustainable quality improvement initiatives. Participates in patient outreach and community engagement and health initiatives to support quality improvement initiatives.
EDUCATION & EXPERIENCE:
Clinical licensure or certification (e.g., LPN or other licensed healthcare professionals) and a minimum of three (3) years of experience in healthcare quality improvement, population health, clinical operations, or a related healthcare setting, or an equivalent combination of education, certification, training and experience. Experience performing clinical chart reviews, quality improvement activities, healthcare analytics, and performance measurement required.
PHYSICAL REQUIREMENTS:
While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle objects; controls, reach with hands and arms; climb stairs; balance; stoop; kneel; talk or hear; taste or smell. The employee must occasionally lift up to 25 pounds. Specific vision abilities required by the job include close vision, distance and color vision, peripheral vision, depth perception and the ability to adjust focus. 50% ambulatory, 50% sedentary.
