Village Point is seeking an experienced MDS Coordinator to join our team!

Village Point is a Springpoint Senior Living Community located in Monroe Township, NJ. We are currently seeking an experienced Full-Time MDS Coordinator to join our team. The MDS Coordinator is responsible for coordinating the RAI process, assuring accuracy, timeliness, and completeness of MDS, CAAs and Interdisciplinary Care Plans, and is responsible for assessing and evaluating the quality of care being given to the residents of the community.

Responsibilities:

  • Determine eligibility of Medicare residents and communicate to nursing personnel the required documentation.

  • Coordinate the RAI process, in conjunction with other members of the interdisciplinary team.

  • Oversee and facilitate the completion of MDS resident assessments. Ensure compliance with Federal and State regulations throughout the MDS assessment and care planning process.

  • Submission of MDS to CMS, and review of validation reports making any necessary corrections.

  • Participate in the development and evaluation of the Resident Care Plan.

  • Assign and order ICD-10 codes according to resident’s condition.

  • Coordinate and participate in the UR and Triple Check processes.

  • Ability to deal tactfully with personnel, residents, family members, visitors and the general public.

  • Exhibit excellent interpersonal relations, time management, family and customer relations and job knowledge.

Education and Experience:

  • High school Diploma or equivalent required

  • 3 years supervisory experience required

  • Must possess a current Nursing license in the state in which the MDS Coordinator is working.

  • RAC Certification Required within one year of hire

  • ICD-10 Coding Certification (AANAC) required within one year of hire

  • Experience in LTC preferred

Salary Range: $80,000 - $83,500

Schedule: 4 days a week, 32 hours a week
