Responsibilities • Deliver occupational therapy services in patients' homes • Complete initial evaluations and follow-up visits • Perform fall risk assessments and home safety evaluations • Develop and implement individualized treatment plans (OT) • Assist with treatment interventions established by the supervising OT (COTA) • Educate patients and caregivers on adaptive techniques and equipment • Monitor patient progress and communicate changes with the interdisciplinary team • Complete accurate and timely clinical documentation Qualifications Occupational Therapist • Active Hawaii Occupational Therapist license • Minimum one year of clinical experience • Home health experience preferred but not required Certified Occupational Therapy Assistant • Active Hawaii Certified Occupational Therapy Assistant license • Minimum one year of clinical experience preferred For Both Positions • Strong communication and documentation skills • Comfortable with community outreach and educational presentations • Valid driver's license and reliable transportation • Ability to work independently while collaborating with the interdisciplinary team Apply Today We're actively interviewing qualified Occupational Therapists and Certified Occupational Therapy Assistants.
