Description:

Trinity Health IHA Medical Group is looking for a detail-oriented and collaborative Ambulatory CDI Specialist to join our Revenue Operations team. In this role, you will partner with providers, coders, and clinical teams to enhance documentation accuracy, support risk adjustment coding initiatives, and provide education that helps improve patient outcomes and organizational success in value-based care programs.

What You'll Do:

  • Review HCC pre-visit documentation and provide coding and documentation recommendations to providers.

  • Analyze patient records to identify potential chronic conditions and documentation opportunities.

  • Ensure diagnosis codes are accurate, complete, and compliant with current risk adjustment coding guidelines.

  • Develop educational materials and provide ongoing training to providers on documentation and coding best practices.

  • Serve as a resource for complex coding questions and provide mentorship to new team members.

  • Support Clinical Condition Documentation program initiatives, performance monitoring, reporting, and strategic goals related to alternative payment models (APMs).

  • Lead and coordinate multiple documentation improvement projects while collaborating with internal and external stakeholders.

  • Identify and escalate documentation and coding trends, issues, and opportunities for process improvement.

General Requirements:

  • EDUCATION: Associate’s degree in business or health care preferred, or an equivalent combination of education and experience.

  • CREDENTIALS/LICENSURE: One of the following certifications required: AAPC (CRC), AAPC (CPC), RHIT. Secondary specialty certification(s) preferred.

  • MINIMUM EXPERIENCE: 2-4 years’ experience in risk adjustment coding preferred.
