Requirements

Key Responsibilities:

  • Conduct ongoing clinical assessments, including OASIS assessments, to identify patient/client needs and determine levels of functioning.

  • Evaluate patients’ occupational performance in areas such as self-care, work, leisure, and rest.

  • Assess sensory-motor neuromuscular, psycho-social, and cognitive-perceptual components of occupational behavior.

  • Evaluate physical, social, and cultural components of patients’ occupational environment.

  • Facilitate direct therapeutic interventions to address ROM, strength, coordination, adaptive equipment needs, and environmental modifications.

  • Develop, evaluate, and revise the plan of care in collaboration with patients, representatives, and caregivers.

  • Document clinical notes, phone orders, and other records in a timely manner.

  • Accept clinical assignments consistent with education and competence.

  • Provide physician-ordered services, including patient/client, caregiver, and family counseling and education.

  • Supervise and guide occupational therapy assistants and home health aides as appropriate.

  • Apply infection control and standard precautions to prevent contamination and transmission of disease.

  • Utilize effective interpersonal relations and communication skills to achieve desirable outcomes.

Job Qualifications:

  • Education:Bachelor of Science in Occupational Therapy.

  • Licensure:Current State License as an Occupational Therapist, Current driver’s license required.

  • Experience:One year of experience as an Occupational Therapist preferred. Home health experience preferred

  • EMR:Home care home base proficiency preferred

  • Skills:Proficiency in therapy skills as per accepted standards of practice, good interpersonal skills, current CPR certification, and knowledge of durable medical equipment.

  • Transportation:Reliable transportation and valid/current auto liability insurance.
