What We Offer

As a Case Manager MSW at Novant Health Matthews Medical Center you will serve as a leader of the patient assessment and complex discharge planning process. You'll work with a wide variety of patient populations, supporting multiple units. Our case managers work as part of a collaborative team of professionals with exceptional training and opportunities for growth. This part-time weekender position offers you the perk of working 24 hours each week and earning 8 hours of incentive pay. A weekender position offers you work/life balance while still giving you part-time benefits. You must be able to orient full time Monday - Friday for a minimum of 4 weeks prior to moving to weekend only training.

What You'll Do

  • Serves as a leader of the complex discharge planning process and a valued member of the healthcare team

  • Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans

  • Collaborate with RN Case Managers for guidance with medically complex cases

  • Serve as a resource for discharge planning and care coordination for psychosocially complex cases

  • Responsible for facilitating coordination of care

  • Maintain current knowledge of available community resources and post acute care options

  • Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health

What We're Looking For

  • Education: Master's Degree in Social Work from school accredited by the Council on Social Work Education, required.

  • Experience: Two or more years professional social work experience or two or more years healthcare experience, strongly preferred.

  • Additional Skills Required: H oliday coverage
