Heartland Home Health is looking for a Full-Time Physical Therapist to care for our patients out of our Midland/Saginaw territory.

What You’ll Do as a Home Health Physical Therapist:

  • Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.

  • Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.

  • Adjust treatment plans and techniques as needed to achieve optimal outcomes.

  • Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.

  • Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.

  • Document all visits, assessments, and progress notes accurately and in a timely manner.

  • Support patients in maximizing their independence and improving quality of life—wherever they call home.

About You

Qualifications – What You’ll Bring:

  • Master’s degree in Physical Therapy from an accredited program.

  • Current PT license in the state of employment (or eligibility to obtain).

  • CPR certification required.

  • Strong communication, assessment, and problem-solving skills.

  • Ability to work independently in a variety of home environments.

  • Valid driver’s license, auto insurance, and reliable transportation.

Preferred Experience (Not Required):

  • 1+ year of experience in a hospital, home health, or outpatient rehab setting

  • In-home therapy delivery

  • Geriatric rehab, orthopedic or neurologic rehab

  • Post-acute care or chronic condition management

  • Working as part of a mobile or interdisciplinary care team
