Position: Case Management Analyst Location Zip Codes; 60607, 60608, 60612, 60622, 60623, 60624, 60639, 60642, 60644, 60647, 60651 This Analyst, Case Management Field position is with Aetna’s Long Term Services & Supports (LTSS) team and is a field-based position in Chicago, Illinois. The requirements is for candidates to travel 50-75% of the time to meet with members face to face. This position holds a full caseload to manage waiver members. This position requires in person home visits with members. This position is critical to meet contractual requirements. Facilitate appropriate healthcare outcomes for waiver/LTSS members by providing care coordination, support and education for members through

Required Qualifications:

  • Minimum of two (2) years of case management experience
  • Microsoft Office and electronic health record experience
  • Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed by our company expense reimbursement policy
  • Valid Illinois Driver’s license and must reside in one of the above zip codes or surrounding areas.

Preferred Qualifications:

  • Case management and discharge planning experience
  • Managed Care experience
  • Microsoft Office experience
  • Experience working with HIV and AIDS population
  • Multi-lingual language proficiency

Required Education:

  • Individual with a bachelor’s degree or a non-licensed individual with a master’s degree, with either degree being in a human-services field (including, but not limited to sociology, special education, rehabilitation counseling)

Pay Range The typical pay range for this role is:

$21.10 - $44.99 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
