Full job description

Job Summary:

In this position, the Social Worker provides comprehensive psycho-social assessment, management, and support to at risk patients and families receives services through Adult Medicine. The Social Worker serves as a resource for consultation, referral, and coordination of services to address the psychosocial needs of the patient population. The Social Worker actively manages patients in collaboration with the multidisciplinary team, coordination of care, monitoring and identifying and connecting patients with appropriate community services. The ideal candidate will demonstrate strong communication, interpersonal and documentation skills, as well as a thorough understanding of Social Drivers of Health and available community resources. The candidate will participate in departmental meetings and in-service education, quality improvement activities and perform other relevant duties as assigned to include cover for other Social Workers and back-up responsibilities.

Minimum Qualifications:

M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required.

*New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Must obtain L.M.S.W. within 12 months of receipt of limited permit.

Preferred Qualifications:

Post MSW relevant experience.

Work Days:

Days with occasional on-call/holiday coverage on rotation based on operational needs.
