The Medication Reconciliation Clinical Pharmacist PRN is responsible for assists in preforming daily pharmacy tasks, including but not limited to: filling orders for medications, monitoring patient therapeutic medications, providing drug information, checking medications, IV mixtures and compounding medications, prior to distribution.
What you will do in this role:

You will utilize good problem solving abilities, making sure that problems are resolved, calling on other knowledgeable parties as necessary.

You will maintain strong knowledge of hospital policies and government regulations and dispense accordingly.

You will consult with physicians and nursing staff regarding patient reactions, errors and complaints.

You will determine and consult on possible drug interactions.
-
You will assist with quality improvement initiatives, inventory and ordering of pharmaceuticals.

You will monitor, record and communicate as appropriate utilizing computerized documentation systems.
-
You will utilize knowledge of age specific needs of patient in performance of duties and responsibilities.

Home Medication List Review and Medication Reconciliation
-
Provides leadership in managing patient-centered medication reconciliation on admission and discharge.
-
Ensures medication history and reconciliation is completed according to established policies, procedures, and protocols.
-
Continuously works with the pharmacy services team and interdisciplinary care team to improve medication history and reconciliation processes.
-
Identifies and resolves drug related problems
-
Verifies each medication documented by the pharmacy technicians in the electronic healthcare record and clarifies questions with the technician and patient as needed.

Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols.
-
Communicates with providers to reconcile the admission medication history taking into consideration equivalent formulary alternatives and drafts the admission medication list for the provider.

Documents interventions in the electronic health record and in CPW/VigiLanz, and cosigns technician notes in the electronic health record.

Ascertains need for language assistance and obtains interpreter as necessary
-
Provides verbal and written medication education to enhance patient understanding.

Identifies and works to resolve drug-related problems that may affect patient adherence.
**What qualifications you will need: **

Graduate of an Accredited School of Pharmacy

Licensure in the State of TN. Board Certification preferred

One-year hospital experience preferred. Residency preferred
TriStar Hendersonville Medical Center is a150 bed hospital that has provided high-quality health care for over 40 years. The facility is the first Accredited Chest Pain Center in Sumner County and a Certified Primary Stroke Center. The Birth Center at TriStar Hendersonville, featuring Sumner County’s only Level II NICU services which hosts spacious LDRP suites and a lactation boutique for inpatient and outpatient services. In addition, our facility is Gold Seal Certified in Total Hip and Total with The Joint Commission.
