Position: Case Management Analyst Location: Field Based - Arlington, Alexandria, Annandale, Springfield, Falls Church Travel: Up to 75%
Position Summary Case Manager Analyst is responsible for face to face and telephonically assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long- term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies.
Required Qualifications
- Must reside in Annandale, Falls Church, Springfield, Arlington, Alexandria VA area
- Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy
- 2 or more years’ experience in behavioral health, social services or appropriate related field
Preferred Qualifications
- Case Management, Discharge Planning and/or Managed Care Experience
Education
- Bachelor's Degree or non-licensed master level clinician required, with either degree being in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling), LMHP, RN/LPN, QMHP, LMSW, LBSW, MSW or BSW.

Pay Range The typical pay range for this role is:
$21.10 - $40.90 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
