Nurse Care Manager and Education Nurse

For Water Street Mission to fulfill its mission and purpose, it is critical for all staff to participate in the development and maintenance of a healthy Christian Community. Even as some positions have minimal direct contact with guests, each staff member has an essential role in forming and maintaining Christian community. Job Summary: Provide nurse care management services and nursing care to Water Street Mission guests and teach and oversee health education classes.

FLSA: Full Time (32 hours; Tuesday-Friday), Non-exempt

All Water Street Mission staff must be:

• Encourage restoration in the lives of coworkers and guests, recognizing we are each restored by God to become restorers.

• Work in full coordination with other WSM staff at all times to identify, report and respond appropriately to any potential threats to the safety and well-being of guests

Role Specific Responsibilities

• Provide NCM services for guests with acute and chronic medical conditions.

• Coordinate referrals to community healthcare providers and support services.

• Maintain accurate, complete, and timely documentation of all care management activities.

• Accompany guests to medical appointments as needed to support care coordination and advocacy.

• Perform physical health assessments for the Residential Program guests.

• Provide oversight for guests in the Physical Health focus.

• Collaborate with Water Street Mission program staff to support guests' health-related needs.

• Participate in multidisciplinary team meetings.

• Oversee Narcan and Spill Kit processes for Water Street Mission departments.

• Perform focused nursing triage and assessments during designated walk-in clinic hours.

• Develop, coordinate, and/or deliver health education classes for guests and staff.

• Perform additional duties as assigned to support program operations.

Education and Experience:

• Associate Degree in Nursing (ADN) accepted; Bachelor of Science in Nursing (BSN) preferred.

• Minimum of 2 years of nursing experience required.

• Current unrestricted Registered Nurse (RN) license in Pennsylvania.

• Experience working with patients who have a history of trauma strongly preferred.

Reports To: Director of Medical Services

Key Internal Relationships: WSHS Team and volunteers, Residential Life, Community Resources, Mission Directors, Key community medical providers
