Job Duties

The Managed Care Case Manager is responsible for conducting comprehensive intake and assessment activities, determining eligibility for long‑term services and supports, and developing person‑centered support plans based on each individual’s assessed needs and preferences. This role coordinates and monitors services, assists clients in accessing appropriate providers and community resources, and ensures follow‑up and reassessment to support ongoing health, safety, and well‑being. The Case Manager collaborates with individuals, families, service providers, and a wide range of professionals to promote health, prevent disease, and advocate for vulnerable populations. Duties also include maintaining accurate case records that meet all regulatory standards, participating in community health assessments and improvement initiatives, delivering culturally appropriate health information, and contributing to quality improvement and performance management activities within the department. Work is performed in office, community, and in‑home settings and requires regular travel as well as participation in public health emergency response efforts.

Minimum Education Required

The minimum education required for this position is a bachelor’s degree in social work, nursing, or a closely related field, along with home and community‑based experience. Candidates may qualify with a bachelor’s degree in social work and at least one year of relevant experience, a bachelor’s degree in nursing with current RN licensure and public health certification plus one year of experience, or a bachelor’s degree in a related field with one year of experience. Alternatively, individuals who are licensed registered nurses without a bachelor’s degree may qualify if they have at least two years of home and community‑based experience.

Minimum Experience Required

The minimum experience required for this position is home and community‑based experience, with the amount depending on the applicant’s educational background. Candidates with a bachelor’s degree in social work, nursing, or a closely related field must have at least one year of experience providing home and community‑based services. Individuals who hold a registered nursing license but do not have a bachelor’s degree may qualify with a minimum of two years of home and community‑based experience. This experience ensures that applicants are familiar with service coordination, client support, and working within community settings.
