CLINICAL proCESS monitor
JOB SUMMARY
As part of a dynamic clinical team embedded in a Health Home Care Coordination Organization, the Clinical Process Monitor is responsible for monitoring clinical/strategic workflows and analyzing the effectiveness of clinical and care coordination interventions, strategies, and processes. The Clinical Process Monitor will examine tasks and processes by gathering and analyzing data, reviewing documentation, and fostering anecdotal feedback to ensure that workflows/processes are designed appropriately, operate effectively and are being completed by the applicable team members. ESSENTIAL FUNCTIONS

Design and perform audits to ensure clinical/strategic tasks and processes are:
-
designed appropriately, effective, and efficient
-
completed accurately and punctually by associated team members
-
compliant with Health Home Care Coordination regulations, policies, and procedures
-
Act as the owner of corrective actions issued regarding clinical/strategic tasks and processes
-
monitor and ensure completion of corrective actions
-
determine the effectiveness of corrective actions
-
assist in their redesign, as needed
-
Create reports of review findings to present to various audiences, including clinical team members, senior management, and care coordination
-
Assist leadership in the redesign of tasks and processes, as needed
-
Provide timely and appropriate feedback to management regarding review findings
-
Maintain strict confidentiality; adhere to all HIPAA guidelines and regulations
-
Maintain quality and proficient services by advancing personal knowledge, skills, and other competencies through continuing professional development
OTHER DUTIES

EDUCATION & EXPERIENCE

At least 3 years of experience, professional and/or personal, with the following required:

Collecting, processing, and interpreting information

Using logical reasoning to evaluate problems and find solutions

Conveying information clearly and accurately to others
-
Acting independently, taking responsibility for one's own work
-
Knowledge of OPWDD, Health Home care coordination preferred
-
Familiarity with person-first, person-centered planning concepts preferred
