About the Role: Tradition Homecare - NY is looking for a compassionate and skilled Private Duty Homecare Nurse to join our dedicated team in Brooklyn, NY. Responsibilities:

• Provide direct, private duty nursing care to patients in their home environment

• Assess, monitor, and document patient health status and progress

• Administer medications, treatments, and medical procedures as prescribed

• Develop and implement individualized patient care plans

• Communicate effectively with patients, families, and healthcare team members

• Educate patients and caregivers on health management and best practices

• Maintain accurate and timely clinical documentation and records

Requirements:

• Active RN license in the state of New York

• Prior home care, private duty, or similar nursing experience preferred

• Strong clinical assessment and critical thinking skills

• Excellent communication and interpersonal skills

• Ability to work independently with minimal supervision

• Reliable transportation and ability to travel to patient homes in Brooklyn, NY

• CPR/BLS certification required
