Description:

Assist patients and families in resolving personal, financial, and environmental difficulties, which predispose illness and interfere with obtaining optimal benefits from health care. Specific functions include psychosocial assessments, treatment planning, financial assistance, counseling, information and referral, advocacy, problem intervention. Perform duties, congruent with NASW Code of Ethics and professional social work standards. Serves patients, families and/or significant others whose ages range from pediatric to geriatric.

Will provide social service consultations and counseling services to all admissions involving; suicide attempts, substance overdoses, and situations involving possible suicide concerns. Counsels patients, family members, friends, public for a wide variety of concerns including depression, grief, adjusting to major life changes, impending death, confused elderly, homelessness, appropriateness of a home situation for a child, and adoptions

ESSENTIAL FUNCTIONS

  • Demonstrates professional responsibility in the role of Social Worker.

  • Complies with personnel policies and hospital safety policies.

  • Maintains confidentiality when interacting with patients, families, personnel and the public.

  • Demonstrates ability to gather timely assessment information including components of home environment situation, support systems, coping skills/stressors, strengths, grief/loss issues, adjustment to illness, identified needs, and goals.

  • Develops and carries-out intervention plan and make appropriate referrals.

MINIMUM QUALIFICATIONS

  • Licensed as a Social Worker or Independent Social Worker through Iowa Dept. of Public Health and the State of Illinois preferred.

  • Must possess a master’s degree from a graduate school of social work accredited by the Council on Social Work Education which included specialization in clinical practice preferred.

  • Bachelor’s Degree in Social Work or related field required.

  • One to two years of social work experience in a health-care related setting preferred.

  • Excellent communication, diplomacy, and people skills required.
