Salary Range - $40.00-$60.00

About the Role:

The Orthopaedic Nurse Navigator plays a key role on the interdisciplinary team as the single point of contact to coordinate care in the hospital setting. The role focuses on patient advocacy, care coordination, disease-specific education and data management to improve outcomes and patient satisfaction during the transitions within the healthcare system. Through patient education at all stages of the care continuum, the Orthopaedic Nurse Navigator bridges the gaps from preoperative preparation, setting expectations of care, and facilitating transition home while improving healthcare literacy and decreasing readmissions.

What You’ll Do:

These requirements are representative, but not all-inclusive, of the knowledge, skill, and ability required of the position. Primary job responsibilities constitute approximately 90% of the positions work. To be successful, individuals must be able to perform each essential responsibility satisfactorily.   • Provide education before and following surgery

  • Coordinate care as the single point of contact/resource for patients, staff and physicians

  • Provide nurse navigation throughout the continuum of care

  • Act as a patient advocate and educator

  • Collect, track, trend, analyze data to improve outcomes

  • Utilize structured care methodologies with pre and post surgery surveys for CMS

  • Promote orthopaedic evidence-based clinical practice

  • Manage criteria/standards for Orthopaedic Center of Excellence

  • Will lead the Orthopaedic Collaborative Committee

  • Will provide support to Pre-procedure Testing (PPT)

What you bring:

NYS RN License. BSN required. Minimum of 5 years relevant clinical experience.

Minimum of 3 years in clinical leadership positions with progressive leadership responsibilities.
