Summary
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
Major duties include but are not limited to the following: The Caregiver Support Program Coordinator develops and implements the PCAFC specific to his/her area of assignment and insures compliance and collaboration with local facility and the VA Central Office Caregiver Support Program initiatives in concert with the facility CSP Program Manager. Participates in direct caregiver and Veteran communication to identify and assess caregiver stress/burden and the development and application of directed clinical treatment interventions. Advises and collaborates with interdisciplinary teams throughout the medical center on caregiver issues. Creates educational tools, develops programs and implements training focused on specific caregiver needs/issues. Provides education and in-service training to VAMC employees, Veterans, families and caregivers. Provides ongoing consultation and support to colleagues regarding caregiver support. Develops and facilitates caregiver support groups (face-to-face or VANTS telephone line) and educational programs. Distributes educational materials provided by VA, local, state and national caregiver organizations. Conducts caregiver and family clinical interventions to facilitate the identification of caregiver stress, psychiatric illness, emotional distress and social needs as treatment issues. Provides referral to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. Serves as the major consultant for information on negotiating the complex processes caregivers, Veterans and families encounter when interacting with government and community agencies. Stays abreast of current federal and state legislation directed at assisting caregivers to continue providing caregiving in an environment that is safe for both them and the Veteran. Maintain a knowledge base of current theory and treatment modalities by reading professional journals, attending conferences, clinic seminars and teleconferences, and materials distributed through VA and private sector sources. Collaborate with the VA Patient Aligned Care Team (PACT), mental health and geriatric primary care clinicians to provide Veteran and staff education on caregiver issues. Maintains advanced knowledge and understanding of the services and benefits available for caregivers from both federal, state and private sector resources. Responsible for developing and establishing linkages and collaborates on VA caregiver support efforts with community healthcare and social service agencies. Administratively responsible and accountable for the clinical and program development and effectiveness of the facility PCAFC. Establish a caregiver support protocol (resources, education and training) sensitive to the issues of the caregivers who are providing care to our youngest returning Veterans. Develop a data monitoring system to measure the efficacy of the General Caregiver Support Program. Collects, analyzes and reports program data to local management, VISN Caregiver Support Coordinator Lead and the National Caregiver Support Program, Care Management and Social Work Service, Office of Patient Care Services. Evaluates program progress and outcomes and utilizes data to continuously modify and improve the program and processes. Basic Requirements: United States Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going tohttp://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. Loss of Licensure or Certification: Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. Physical Requirements: See VA Directive and Handbook 5019, Employee Occupational Health Services. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Senior Social Worker, GS-12 Experience and Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: Knowledge of program coordination and administration which includes : consultation, negotiation, and monitoring. One Year Case Management Experience (care coordination, discharge planning, interdisciplinary team collaboration, etc. One Year Experience using electronic charting systems, include secure messaging and video tele-health systems. Physical Requirements: The work is sedentary. Some work may require movement between offices, hospitals, warehouses, and similar areas for meetings and to conduct work. Work may also require walking/standing, in conjunctions with travel to and attendance at meetings and/or conferences away from the work site. Incumbent may carry and lift light items weighing less than 15 pounds. Functional Requirements: Individuals, including wage-grade drivers, title 5, title 38, and hybrid title 38 employees or volunteers who are required to operate a government owned or leased vehicle to successfully carry out their assigned duties must be medically cleared prior to appointment (positions that require a CDL are covered under a separate OF-178). Assessment must be made for any acute or chronic medical/physical condition or medication use which interferes with the ability operate the appropriate Government-owned or -leased vehicle safely and without undue risk to themselves or others. The following requirements must be met: distant vision 20/40 in one eye with or without correction, field of vision 70 degrees, ability to distinguish red, green and amber, whispered voice at five feet, or average hearing loss of not greater than 40 dbs. at 500, 1000 and 2,000 Hz.
