Full job description
We are seeking a licensed Physical Therapist (PT) to join our skilled nursing team in Marion, SC. This is an excellent opportunity for a clinician who values patient-centered care, clinical autonomy, and collaboration within a supportive interdisciplinary environment.
Position Summary
The Physical Therapist evaluates, plans, and implements individualized treatment programs for residents in a skilled nursing setting, helping patients regain strength, mobility, and functional independence following illness, injury, or surgery.
Key Responsibilities

Perform comprehensive physical therapy evaluations and develop individualized treatment plans

Provide skilled PT services to short-term rehab and long-term care residents

Establish measurable goals and monitor patient progress

Document evaluations, treatment notes, and progress reports in accordance with regulatory standards

Collaborate with physicians, nursing staff, therapy team, and families to optimize patient outcomes

Participate in care plan meetings and discharge planning

Ensure compliance with Medicare, Medicaid, and facility policies
Qualifications

Current Physical Therapist license in the State of South Carolina

Graduate of an accredited Physical Therapy program

Experience in a skilled nursing or rehab setting preferred (new grads encouraged to apply)

Strong clinical, documentation, and communication skills

Ability to work independently while contributing to a team-based environment
