LICENSED PRACTICAL NURSE (LPN) - PALLIATIVE CARE COORDINATOR - HOSPICE SERVICES

KEY RESPONSIBILITIES:

  • Responsible for taking and processing Palliative Care referrals.

  • Provide support to the Palliative Care Nurse Practitioner and Medical Director.

  • Will schedule visits to support goals of care as ordered by MD.

  • Coordinate and communicate patient care goals between all providers.

  • Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.

  • Ensure communication of assessment and reassessment findings to attending physician.

  • Complete all documentation accurately, timely, and in accordance with company policy and procedure.

  • Inform palliative team of all new admissions and discharges.

  • Coordinate referrals utilizing the referral process.

  • Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.

  • Run billing report and address validation holds/ billing errors

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS:

  • Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN) licensure in the state of practice

  • A minimum of three (3) years of nursing experience

  • Palliative care, hospice, or home health care experience is preferred

  • CHPN (Certified Hospice and Palliative Nurse) is preferred
