About the Role: ComForCare Home Health Care (North San Diego) is looking for a compassionate and dedicated Direct Support Professional to join our growing team in San Diego, CA. In this rewarding role, you'll make a meaningful difference in the lives of individuals by providing personalized care and support right in the comfort of their own homes. Responsibilities:

Provide hands-on personal care assistance including bathing, grooming, dressing, and hygiene support

Assist clients with daily living activities (ADLs) to promote independence and dignity
-
Offer companionship and emotional support to clients and their families
-
Assist with meal preparation, light housekeeping, and errands
-
Monitor and document changes in client health, behavior, or condition and report to supervisors
-
Safely assist with mobility, transfers, and ambulation as needed
-
Administer medication reminders in accordance with care plans
-
Follow individualized care plans to meet each client's unique physical and emotional needs Requirements:
-
Previous experience as a Direct Support Professional, caregiver, home health aide, or similar role preferred

Genuine compassion, patience, and dedication to supporting individuals in need
-
Ability to pass a background check and drug screening

Valid driver's license and reliable transportation

Strong communication and interpersonal skills

Ability to work independently and follow detailed care plans
-
CPR/First Aid certification (or willingness to obtain)
