Equinox is Albany County’s primary provider of services for victims of domestic violence. Our Domestic Violence Programs provide safety for victims of Domestic Violence, including the elderly, and offer the support, assistance and tools they need to escape abusive situations and take control of their lives. Domestic Violence Services are available to all victims and their dependent children, regardless of gender, age, ethnicity, or sexual orientation.

Position Summary:The Case Manager provides individualized case management to clients in the Project Break Free rapid re-housing program. Services provided include finding safe and affordable permanent housing, connecting client to needed services, DSS and other service accompaniment, liaison services, information, assessment, education and support. The case manager will assist clients with credit repair, budgeting and other financial education.

Essential Duties and Responsibilities:

  • Assesses client needs and abilities, determining level of support needed to provide adequate assistance.

  • Accompanies clients to DSS, court, law enforcement, other agencies as needed; advocates for clients both in person and through phone and written contact.

  • Maintains accurate case records; completes necessary paperwork and statistic requirements for the agency and funding sources.

  • Develops safety plans with clients.

  • Assists clients in finding safe and affordable permanent and/or temporary housing and moving into the same.

  • Develops and maintains positive working relationships with community services and landlords.

  • Participates in staff meetings, as well as individual supervision.

  • Reports suspected cases of child abuse/maltreatment as a mandated reporter.

  • Acts in accordance with Equinox and Domestic Violence Services Program mission.

  • Ability to model Crisis intervention skills.

  • Ability to provide objective supportive listening and guidance.

  • Ability to write and speak clearly and concisely.

  • Ability to work independently, as well as part of a team.

  • Ability to model calm and cooperative behavior.

  • Ability to prioritize and complete multiple tasks in an organized manner.

  • Ability to maintain strict client confidentiality and engage with a diverse population.

  • Performs other duties as requested for the enhancement and continuation of programming

Requirements

Physical Requirements:

  • Ability to negotiate stairs.

  • Ability to lift and carry minimum of 15 lbs.

  • Ability to handle basic facility emergencies.

  • Ability to drive and transport clients with agency vehicles.

Minimum Qualifications:

  • High School Diploma or equivalent with a minimum of four years’ experience in human services.

  • Must have a clean, valid NYS Driver License, with access to reliable transportation.

Preferred Qualifications:

  • Associates’ Degree in a human service-related field preferred with a minimum of 3 years’ experience in human services OR a Bachelor’s Degree in a human service-related field preferred with a minimum of 2 years’ experience in human services.

  • Fluency in Spanish and/or an additional language.

  • One year case management/crisis intervention experience

  • Prior experience working with domestic violence, sexual assault, elder abuse, human trafficking, mental health or substance abuse populations preferred.

  • Knowledge of crime victims’ compensation, state domestic violence regulations, domestic violence dynamics and DSS regulations.
