Job Description

I. Position Summary: Responsible for securing rental and agency-owned housing. Works with staff to ensure housing stability and permanence for consumers. Perform case management duties and supportive counseling.

II. Essential Responsibilities:

  1. Provide mediation and advocacy with landlords on the client’s behalf to develop a plan to obtain and or maintain housing.

  2. Assist clients in locating and securing housing of their choice.

  3. Serve as an ongoing liaison between property managers and clients.

  4. Ensure that a minimum of 30% of work hours are spent rendering face to face Community Support Services to consumers.

  5. Provide pro-active follow-up home visits to ensure stability and further progress towards self-sufficiency: this includes support, advocacy, listening, and problem solving.

  6. Coordinate and facilitate furniture ordering and delivery.

  7. Provides education on their rights and responsibilities as a tenant and provide education of guidelines on specific subsidies.

  8. Ensures proper maintenance of all facilities.

  9. Works with the county, state, and federal housing authorities on rental assistance issues.

  10. Provides case management services, participates in development of service plans and assists consumers in following prescribed plans.

  11. Oversees the safety of consumers in their apartments.

  12. Utilizes crisis intervention skills when needed.

  13. Participates in other job responsibilities and functions as deemed appropriate by the supervisor.
