Reports To: Director of Staff Development and Medical Supports

Job Summary:

The Medical Liaison is an integral part of the treatment team in the site. This position works with adults with developmental disabilites. The Medical Liaison is responsible for assisting the Clinical RN and the site Management team to ensure that each individual’s medical needs are met and that required medical follow up is completed.

Essential Job Duties:

  • Assists the Clinical RN in all aspects of medical follow up for the individuals served.

  • Schedules all necessary/recommended medical appointments, documenting such in Therap and generating an accompanying clinic consultation sheet.

  • Attends scheduled medical appointments as deemed appropriate by the RN, PC, or RM.

  • Upon return from attended appointments will ensure that all recommendations, order changes and follow up is completed/scheduled, communicating such to the RN and PC and documenting such in Therap.

  • Initiates Medical Monitoring as per RN/MD directive.

  • Assists RN with lab scheduling, completion and obtaining of reports.

  • Completes medication orders with the pharmacy twice a week, ensuring that all MD orders/scripts are up to date.

  • Participates in the weekly site team meeting to ensure ongoing communication with the Clinical RN, Program Coordinator and Residence Manager.

  • Assists the RM with the “Daily MAR and BM Record Check” completion.

  • Assists with individual care, as necessary.

  • Perform any other duties listed on the daily/weekly/monthly/semi-annually/annually site specific responsibility checklist.

  • Along with the management team, ensures the site is safe, clean and supports the individuals.

  • Any other duty as assigned by your Supervisor.

Qualifications:

Minimum Education Required:

-Minimum Degree Required: High School Diploma/GED

Minimum Experience Required:

-None

License/Registration/ Certifications Required:

-NYS Driver’s License

Training Requirements:

  • New Hire Orientation

  • OPWDD Medication Administration Course

Physical Requirements:

•Strength: Sufficient to assist with lifting and transferring a patient, and perform CPR

•Mobility: Sufficient mobility to bend, stoop, and bend down to the floor; ability to move around rapidly; and to move in small, confined areas.

•Hearing: Sufficient to hear through the stethoscope to discriminate sounds; to hear cries for help; to hear alarms on equipment and emergency signals; and various overhead pages.

•Vision: Sufficient to make physical assessments of patients and equipment.

•Communication: Able to communicate in both verbal and written formats; and interact with clients, staff, and faculty supervisors.
