The Care Coordination Assistant Responsibilities Include:

Entering new referrals and updates all existing patient information in corporate IS systems including demographics, insurance, and benefit information.

Verifying insurance and benefits for all Medicare, Managed Care and Private Fee for Service Plans. Securing authorization for services provided by BayCare HomeCare from the payers.

Selecting the correct payer codes to provide accurate intake information.

Understanding medical criteria for securing all required information from the referral source.

Entering the appropriate financial information including benefits, deductibles, co-pays, coinsurance, out-of-pocket, another related costs into the EMR system.

Timely requesting and communicating of authorizations to payer and documentation in EMR.

The Care Coordination Assistant will work under the direction of the Care Transition Coordinator or RN Case Manager to establish and maintain a positive working relationship with patients, payors and other customers.

Education:

Required High School Graduate or Equivalent GED

Preferred Technical Diploma – Medical Specialization
