Experience: 1 year, will consider less experience for the right candidate
License, Registration and/or Certification Requirement: Must Possess a current New York State Licensed Practical Nurse (LPN) designation
Minimum Knowledge: Requires knowledge and previous experience in med pass, use of electronic medical records (EMR), supervision of staff as well as geriatric care
Functions
-
Provides safe resident care, demonstrating knowledge and competency in completing assigned treatments and procedures according to established standards and policies. Maintains awareness of new updates standards and incorporates them into his/her nursing practice through out the continuum of care.

Participates in the improvement process of quality resident care.
-
Incorporates age-specific needs, provision of care, procedures, and treatments.
-
Accurately completes routine tasks without the need of supervision.
-
Communicates pertinent resident information and plan of care before breaks, during transfer, and at shift report.

Assists residents with self-administration of medication as per physician orders and accurately documents this in the medical record
-
Directly Supervises Home Health Aides and ensures that resident care is provided timely and in accordance with NYS regulations
-Completes reports/audits as assigned by Clinical Director and/or Administrator
-Assists the community with new resident admissions/readmissions. Helps residents and families acclimate to the community by providing guidance and emotional support
·Ability to recognize a change in resident condition and Notifies physicians and family members of resident(s) change
- Attends and completes all required staff training/in-services
-Assists in maintaining a calm, homelike environment for the residents
-Assists Home Health Aides in the delivery of resident care as needed

Completes resident care-related activities within scheduled shift time periods.

Exercises calm behavior and uses professional judgment while working in a team approach during emergency situations.

Cooperates with reassignments made in the interest of resident care.

Maintains positive interpersonal relationships with colleagues.

Professional conduct and behavior is exhibited by self-control, cooperative interaction with others, and acceptance of responsibility for own actions.

Reports resident care problems and possible solutions to the appropriate nursing management personnel.

Delegates resident care problems and possible solutions to the appropriate nursing management personnel.

Performs other duties as assigned
