Salary Range $17.00 - $18.00 Hourly

The Activity Support Provider assists with the day-to-day operations of the Day Services Program. They carry out responsibilities in the following functional areas: goal setting, independence, community involvement, consumer rights, personal cares, daily living activities, and regulatory compliance.

Key Responsibilities:

Support members in community and recreational activities

Assist members with physical, social, and personal needs

Guide members in achieving personal goals and decision-making

Create fun and engaging activities tailored to member interests

Prepare and serve meals following dietary needs and preferences
Education/Qualifications:
-
Must possess a high school diploma/equivalent and/or related work/life experience.

Must be at least 18 years of age.

Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.

Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
