Full job description

JOB SUMMARY: A paraprofessional person who is specifically trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN).

QUALIFICATIONS:

  • High school diploma, preferred.

  • Current driver's license.

  • Must meet Medicare Conditions of Participation for Home Health Aide training program and

  • Able to read, write and comprehend English.

  • Have a sympathetic attitude toward the care of the sick and

  • Ability to carry out directions, read and

  • Maturity and ability to deal effectively with the demands of the

  • At least one year of experience preferred as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health agency. Must have successfully completed a state-established or other training and competency evaluation program that meets state and federal requirements, or a state licensure program that meets state and federal requirements for aide training and competency evaluation program, or a competency evaluation program if, since the individual’s most recent completion of this program(s), there has been a continuous period of 24 consecutive months during none of which the individual furnished home health aide services described in 42 CR 409.45 for compensation.

RESPONSIBILITIES:

  • Understands and adheres to established Agency policies and

  • Performs personal care, bath and hands-on care as

  • Completes appropriate visit records in a timely manner as per Agency

  • Reports changes in the patient’s condition and needs to the

  • Performs household services essential to health care in the home as

  • Ambulates and exercises the patient as

  • Performs simple procedures as an extension of the therapy or nursing services, g., range of motion (ROM) exercises as assigned.

  • Assists with medications that are ordinarily self-administered as

  • Attends inservice and continuing education programs as scheduled and

  • Attends patient care conferences as

WORKING ENVIRONMENT: Works indoors in Agency office and patient homes and travels to/from patient homes.
